Commonly Used Forms
You may be required to submit documentation to the Financial Aid Office to complete your financial aid. Certain forms requested can be found below. The Financial Aid Office will let you know via your CCC email which forms you need to complete.
There are a variety of ways you can submit information to our office. Type the information into the form, print, and sign prior to submitting; any unsigned forms will be returned to you for signature. You may submit the forms in the following ways:
All forms (except the Identity/Statement of Purpose Form and Signature Page) for the 2023-24 academic year can be submitted through DocuSign. DocuSign is an application that allows students and parents/guardians to securely complete and sign forms digitally, eliminating the need to print and scan PDF documents.
Email validation is required for all forms. Once all signatures have been submitted, DocuSign forms are routed back to our office and will be attached to your student record. Please allow 1-3 days for your checklist to reflect completed forms.

Regardless of the campus you are attending you can turn documents in to any of the Financial Aid Offices located in Columbus, Grand Island, Hastings, Kearney, Lexington, Holdrege, or Ord.
